I'm Penny Krautbauer. I help families find the right care, at the right time, for the right reasons.
I'm the founder of Transitions in Senior Care and a Certified Senior Transition Specialist. For 20+ years in the senior-care industry, I've been helping families across the Albuquerque and Rio Rancho area navigate the often overwhelming process of finding the right senior care and living options.
I help families understand their options, make informed decisions, and transition their loved ones into the appropriate level of care, with less stress and greater confidence.
My why
Why I do this work
I didn't get into senior care simply because I saw a need. I got into it because I genuinely care about people and the families who love them.
I've seen how emotional and confusing it can be when a family realizes that Mom or Dad may need more support. There are so many questions: What do we do next? Where should they live? Who can we trust? How do we know we're making the right decision?
I wanted to be the person families could call when they didn't know where to start. Someone who would listen first, understand their unique situation, and help them find a path forward without pressure or judgment.
What continues to drive me is the people. I love hearing their stories, getting to know what makes each senior unique, and helping families find a place where their loved one can be safe, cared for, and truly feel at home.
Behind every transition is a person, a family, and a story, and being able to make that journey a little easier is incredibly meaningful to me.

My mission is to make the journey into senior care less overwhelming and more manageable for families. I'm here to listen, educate, and guide families through their options so they can make decisions they feel good about, while helping their loved ones transition into a place where they are safe, supported, and cared for.
I want families to feel heard, never pressured. My role is to educate, guide, and simplify the process, not make the decision for them.

How do I know when it's time for more help?
If everyday tasks are getting harder, a fall has happened, memory problems are affecting safety, or the family caregiver is running on empty, it may be time to explore options while there is still room to plan calmly.
When a family says "we don't know what to do," I want them to think: "Call Penny. She'll help us figure it out." That is the role I'm honored to play, and it's why every family who works with me gets personal attention, local knowledge, and honest, compassionate guidance.
